A laparotomy instrument set is the core tray for open abdominal surgery. It is larger than a minor set and includes longer instruments to reach deep into the abdomen. Every hospital adjusts its set to its own surgeons and procedures, but most are built from the same groups of instruments.

Note

This is a general overview to help with planning and ordering. Your set should be agreed with your surgeons and sterile services team.

Instruments by function#

Function Typical instruments
Cutting Scalpel handles No. 3 and No. 4; Mayo scissors (straight and curved); Metzenbaum scissors (standard and long)
Grasping Toothed and non-toothed tissue forceps; DeBakey forceps (medium and long); Allis and Babcock forceps
Clamping Mosquito, Kelly or Crile, and Rochester-Pean hemostats; Kocher forceps; right-angle (Mixter) forceps
Retracting Richardson, Deaver and Army-Navy retractors; a self-retaining frame retractor such as Balfour
Suturing Mayo-Hegar needle holders in two lengths
Suction and swabs Poole and Yankauer suction tubes; sponge-holding forceps
Drapes Towel clamps

Longer instruments for depth#

Abdominal surgery often works deep in the body, so a laparotomy instrument set includes longer versions of scissors, forceps, needle holders and clamps alongside standard lengths. Having both avoids awkward reaching with short instruments or unwieldy use of long ones near the surface.

Designing the set#

Set design checklist

  • Start from the procedures the set is used for, not a catalogue list.
  • Remove instruments that are rarely used; offer them as separate supplementary packs.
  • Check the total weight against sterile services limits.
  • Use a tray list with photos and consistent layout.
  • Keep spares of the instruments that wear fastest, such as scissors.

Frequently asked questions#

How many instruments are in a laparotomy set?#

It varies widely, often from around 50 to over 100 items depending on the hospital and procedures.

Should laparotomy sets include a self-retaining retractor?#

Many do, because it frees an assistant. Large frame retractors are sometimes packed separately because of their weight.

Can sets be standardised across hospitals?#

Standard core sets with optional supplementary packs often work well and reduce reprocessing costs.
